WANTED nurse practitioner for seniors pain refills and monthly visits

My old nurse practitioner moved out of state and at 71 years I have been
98% homebound last 3 years and need a new prescription and monthly visits if needed
at 60 yrs about 10+ years ago my XRAY came back and I was told lower back arthritis.
